Acute Management Of Peristomal Skin Injury

Inability to maintain a seal on an ileostomy pouch presents a significant issue for the ileostomate. Peristomal skin irritation results quickly causing significant pain, frustration, and embarrassment. The patient to be presented in this poster represented several unique challenges: her stoma was at skin level,the location of the stoma, obese patient, and the stoma spicket pointed upward. After failure of multiple pouches to maintain a seal, the patient presented with completely denuded peristomal skin. The Miller Drain and negative pressure* was attempted in an effort to contain the liquid effluent and allow healing of the denuded skin. The patient and her family were able to maintain the system containing the effluent and allow healing of the skin. After two weeks, the patient's appetite had improved, the effluent had thickened and the skin healed. We were then successful with a pouching system. *Blue Sky Negative Pressure System
